Clinical Surveys for Subjective Measures

With Clinical Surveys, practitioners can collect information from a patient or client before each scheduled appointment. The answers to survey questions are added to the chart, much like Jane’s intake forms.

In this guide, we’ll cover another great use for Clinical Surveys: collecting subjective measures before appointments!

⚠️ Heads up - information from the survey is entered directly into the patient chart. This survey is directed towards patient care. We have a separate feature that enables patients to provide ratings & reviews. You can learn more about that over here.

Here’s a quick example! You’ll notice in the image below that a survey looks similar to an intake form, however, it simply includes your own custom questionnaire, and does not require the client to complete any profile information or consent fields.

Here’s how you can get started!

Creating the Survey

To set up a new Survey, full access users can go to Settings, select the Forms & Surveys tab, select View beside Clinical Surveys and then click New Survey.

Here is where staff can customize the Survey Name and decide if this should be sent on a per patient basis or to all clients.

This is also where you would schedule exactly when the survey should be sent to clients, and toward which type of appointment, in-person, virtual or all appointment types.

📝 While the settings set for the survey here will be seen as a default, you are able to customize the Setup & Scheduling of the survey on a per patient or client basis, right in their profile!

Adding your Survey & customizing the email

The latter half of the Clinical Survey provides the clinic a space to add in the Title/Email subject, as well as an Introduction Message & Message After Completion.

📝 While you can build this from scratch, similar to your Intake Forms you’ll be able to access the Template Library and utilize either your own chart templates or other surveys created by different Jane users!

When you’re satisfied with your survey, hit Save at the bottom of the page to ensure your changes stick.

This will also enable the survey to be sent to your patients prior to future appointments.

Patient View

Patients will receive a link to the Clinical Survey on the day and time you specified. From here, they can click the Begin button.

They will be taken to a dedicated page in their account where they can fill out the form. This form is only accessible from the email and is not currently accessible from within their account.

Once the patient fills out the survey, it will save directly to their chart, and their answers will also be made available to them inside their My Account portal, under the Documents section.
