How to Populate a Fillable PDF File

So you’ve got your fillable form all ready to go and it’s time to see the magic happen— you’re in the right place!

This guide will go over how to populate a configured fillable form for a patient appointment, review that the information entered, and how to download or print this form for submission.

A friendly reminder that this feature will help pre-fill the paperwork to give you a bit of a head start. There may still be certain fields that are unavailable in Jane and will have to be filled out separately. As well, this feature will not directly submit this paperwork to the insurance company— you’ll want to make sure that you download or print the document and send that over to them.

Populating a Fillable PDF Form

To get started, click on the particular insured appointment that you are working with in the Schedule, or from the patient’s Profile > Appointments area. The Appointments Panel should appear on the right-hand side, and we’re going to want to navigate down to the Insurance Info section.

📍NOTE: You’ll want to make sure that this appointment has been insured, otherwise your Fillable Forms will not be available to select. You will be able to Add a Claim/Policy from this same Insurance Info section— here is our guide on Booking an Insured Visit for a detailed walkthrough of this workflow.

From here, click on the arrow (in the circular button) on the right and select Export File.

You should now see a list of all of the fillable PDFs related to that insurer that have been set up— find the form that you are required to fill out for this patient. If you do not see any forms listed listed here, you’ll want to make sure that your fillable form has been (1) enabled and (2) assigned to the correct third-party insurance company.

Click Export & Print and a new browser tab will open with the proper fields pre-filled. You can still edit the file directly from here, as well as save the file to your local desktop or print it out.
